The operating room is data-rich and insight-poor. Devices run in isolation, splitting the surgeon's attention exactly when it matters most. XRlabs brings Surgical Intelligence into the OR: a device-agnostic AI software layer that runs on the equipment hospitals already own, unifies every device into one anatomy-aware view, and lays the foundation for our surgical co-pilot.
of operative time is lost to manual scope adjustment alone, in one of the highest cost-per-minute settings in the hospital.
Eivazi et al., Acta Neurochirurgica, 2015 →Surgeons build intuition by ascending a set curriculum: anatomy, tissue and disease, surgical access, live pathology, and operative skill. We're building Surgical Intelligence the same way, layer by layer, starting in microsurgery. Every XRlabs product is a layer of the pyramid.
See our approach →XRlabs provides a new dimension of insight into patient anatomy: deep vision. It elevates planning, brings anatomy into the operative view, and accelerates skill building.

XRlabs SIM reconstructs standard CT and MRI scans into interactive 3D digital twins in an extended reality environment. Surgeons can visualize anatomy with clarity, plan complex procedures, and rehearse in real time.
Explore XRlabs SIM →SurgicalOS brings the digital twin into the operative view, fusing live video from the surgical scope, navigation and intraoperative imaging into one anatomy-aware picture, on the displays surgeons already use.
Explore SurgicalOS →In December 2025, SurgicalOS was evaluated in a live operating room for the first time: the first in-human use of Physical AI in complex cranial neurosurgery.
Founded by neurosurgeon Dr. Ali Haddad, XRlabs was born from real OR experience. Working closely with clinicians and medical device partners, we are a multidisciplinary team united by a patient-first mission and a relentless drive to advance surgery.
Our company →See how surgeons are already using XRlabs, from groundbreaking procedures to next-generation medical training.
A true surgical foundation model must learn the way surgeons learn. Surgeons do not acquire judgment by watching video alone; they ascend a curriculum. Anatomy first. Then tissue properties and disease. Then surgical access. Then live pathology in the operating room. Finally, operative skill. Each layer gives meaning to the one above it. Together, the layers become what surgeons call intuition. Encoded, they become Surgical Intelligence.
Only the top of the pyramid.
The whole pyramid, trained layer by layer.
Surgical video alone shows what surgery looks like. It does not show where the anatomy is relative to the patient's own imaging, because ordinary video carries no spatial registration.
Every case captured through SurgicalOS produces scope video registered, frame by frame, to the patient's digital twin, alongside navigation telemetry and intraoperative imaging. That record is the training substrate for the top layers of the pyramid: live pathology and surgical skill can only be learned where they happen. It exists only because of the device integrations that create it.
Each deployment adds to it and strengthens the intelligence behind our surgical co-pilot. Each model trained on it makes the next integration more capable. That is how Surgical Intelligence compounds.

Surgeons explore each patient's anatomy in detail through a lightweight headset. This hands-on preparation enables clearer preoperative planning, enhanced intraoperative confidence, and more precise preparation for the approach.
Using a patient's own 3D anatomy model, surgeons can visually guide patients through their upcoming procedure, step by step. It improves comprehension, builds trust and transparency, and increases confidence in the care process.
With interactive 3D anatomical environments and virtual surgical scenarios, trainees can visualize and manipulate anatomical structures, practice in a safe, immersive setting, and develop confidence through realistic, hands-on experience.
In a qualitative study at Imperial College School of Medicine, students taught neuroanatomy with XRlabs SIM alongside cadaveric teaching described the two as complementary, and valued SIM for visualizing deep or otherwise obscured structures such as the ventricular system and cranial nerves.
Kasbia et al., Anatomical Sciences Education, 2026 · open access →XRlabs SIM is layers one to three of the Pyramid of Surgical Intelligence. The digital twin built here is the same twin SurgicalOS carries into the operative view, so planning and operating become one continuous, anatomy-aware workflow.
Explore SurgicalOS →Switching between the operative field and separate monitors can degrade hand-eye coordination.
Gsaxner et al., 2023 →Guidance presented directly in the surgeon's field of view significantly reduced load in a controlled 25-participant study.
Herrlich et al., 2017 →SurgicalOS sits underneath the devices in theatre as a common intelligence layer.
The patient's anatomy, the ground truth every device is read against.
What every connected device sees: scope video, navigation, intraoperative imaging.
New devices join the same environment. The more that connects, the more each part knows.
SurgicalOS unifies whatever the operating room produces, from live scope video and navigation telemetry to intraoperative imaging and the next device to arrive, with the patient's digital twin, built in XRlabs SIM, into a single anatomy-aware view, displayed on the scope's native monitor by default, or through an extended reality headset when stereoscopic depth is preferred. No new hardware footprint. No new real estate in the OR.
SurgicalOS reads the scope's existing video feed without sitting in its path, so no software event can interrupt it. Standard displays and navigation systems remain in use throughout.
During a complex cranial procedure at a leading academic medical center, the system's computer vision tracked the surgeon's instruments and repositioned the surgical scope to keep the field centered and in focus, under the surgeon's direct supervision and control, with manual override available at all times.
A patient-specific model of critical structures was overlaid onto the live microsurgical view as the team navigated a complex skull base corridor, with the hospital's standard navigation remaining the sole source of clinical guidance throughout.
Every case captured through SurgicalOS produces surgical video registered to the patient's anatomy: the training substrate for the layers of the pyramid that only exist in the operating room.

The scope, navigation and imaging you already sell stay exactly as they are — SurgicalOS runs alongside them.
Built to sit beside your device, not replace it: a modular, API-friendly architecture that connects to what you already ship.
Connects to scopes, navigation, imaging and robotic platforms through their existing outputs
New device protocols supported through partner integrations, not firmware changes
Your hardware, protocol and primary display remain exactly as they are
One box, beside your device. It reads the scope's existing video feed and returns an anatomy-aware view — your hardware, protocol and primary display are untouched. No new footprint on the floor, no new screen for the surgeon.
The business case for an OEM, not a feature list.
Bring new AI-enabled capabilities to compatible systems already in theatre, without asking the hospital or your team to replace hardware.
Introduce new software and AI capabilities faster, instead of building and maintaining a proprietary software stack for every device.
SurgicalOS enhances your device, your workflow and your brand. It does not compete with them.
Your device keeps gaining capabilities over time, setting existing and future systems apart beyond hardware specifications.
An integrated device becomes part of the SurgicalOS environment as it grows, where visualisation, navigation, imaging and patient data increasingly work together.
Your hardware, protocol and primary display stay intact. XRlabs provides the intelligence layer around them, and the surgeon still sees your screen.
XRlabs develops its integrations as Software as a Medical Device and can lead the pathway or support yours, depending on the integration and the partnership structure agreed. Either way, partners gain new capability without building and validating a separate solution for every device.
XRlabs is partnering with device manufacturers to make the devices already in theatre smarter, and to grow one connected environment around them.
Clinicians, immersive engineers, AI researchers and regulatory specialists, driven by a patient-first ethos. We are building a visualisation platform surgeons want to wear and a brand they can trust. That trust begins with how we work with each other.
Founded by Ali Haddad, a neurosurgeon, XRlabs has supported landmark cases, including the separation of craniopagus twins, and deployments across NHS trusts, the US and the Middle East.

Three words for how we behave, not what we say. They are the behaviours we reward, repeat and refuse to work without.
Pursue what others call impossible.
We keep a healthy disregard for the impossible: Surgical Intelligence in every operating room, accessible worldwide.
Run towards the hard problems and own them.
No blame, no hiding mistakes, no waiting for permission. When things go wrong we ask why and fix it together.
Finish the job, and be proud of how it was built.
Innovation is only innovation if it measurably improves patient outcomes. Everything we ship must earn its place in the operating room.
Headquartered in London, close to the NHS trusts and academic centres where XRlabs SIM and SurgicalOS are used and evaluated. Our Palo Alto office works with US health systems and device partners.
Your work reaches patients. These are the rituals and terms that make that possible.
After every deployment, the clinical team presents what happened in theatre and how each person's work mapped to it.
